Specialist Care

including Dementia and Long Term Conditions

Partners in Care provide support for the people with long term conditions, those who are living with dementia and those who need palliative or end of life care.

The registered manager who is a registered nurse has completed a five day foundation in palliative care course at St Catherine’s Hospice and our Care Workers will all receive training through the hospice.

Caring for someone with Dementia can be challenging and so we ensure we have a full understanding of our clients and their families unique circumstances. Our care packages in this regard are tailored to the individual which promotes continuity.

We are members of the Dementia Action Alliance and provide training for our care workers to ensure they know what best practice looks like in dementia care.  We are committed to providing high quality care to people living with dementia in line with good practice guidance.

Our care workers will;

  • Ensure a safe environment
  • Positive behaviour support where appropriate
  • Provide nutritious meals
  • Engage with people in stimulating activities
  • Create interesting social interactions
  • Supervise and co-ordinate daily activities

One of the main aims of our care workers is to provide the correct level of support to be able to enhance the simple pleasures in life and help to remove any stress and tension.

We want to provide an opportunity for people to follow as near as normal patterns of daily life as possible and assist them in maintaining a good quality of life.
